Chronic conditions are on the rise, and the health care sector has not adequately responded to the changing needs of patients with chronic illnesses, who require complex care regimens and often leave the doctor’s office with little information on how to properly self-manage their conditions. The community health worker care model can fill this gap. These models seek to hire health coaches locally and position them to motivate and teach patients to self-manage their conditions, navigate the health care system, and connect with social services. Highlighting work with Janice Nittoli Practitioner Fellow Manmeet Kaur, CEO and Founder of City Health Works, this convening will explore how to better care for patients with chronic illnesses and address their complex needs outside the doctor’s office. 

Former US Secretary of the Department of Health and Human Services, Kathleen Sebelius, will discuss the system-level implications of the work and provide closing remarks.
